This routine is for name patterns in any form.
The first pair of GVN-NM1 or ALPHA attributes with the same original value is turned into a SURNAME attribute, and all others that match it.
Note:
This routine could replace routines NMF01, 02 and 03 but is riskier, and should be used more sparingly.
