Forms must be created and customized in order to build a project. Individual forms can be created based on the requirements of the project.

Widgets

Field users can enter data and interact with the program using form widgets.

You can use any of these widgets to design a field survey form based on data collection requirements. The software also enables you to design conditional forms.

Field staff will be assigned forms that will enable them to collect data.

Every widget consists of two default parameters: Label and Question code. Each parameter will have a label displayed in the mobile application. The question code will be displayed in the table where the Admin or Analyst will approve, reject, or send the data for resurvey. This same question code will appear in the downloaded excel file.

Note:
  • Question code must have minimum of three alphabets.
  • Space and special characters such as !, @, #, $,%, &, *, (,), -, +, are not accepted in the question code. It only allows the character "_" (underscore).
  • Label name must not conflict with the labels of other widgets.
  • The same label name and question code can also be used.

Furthermore, all individual widgets include the following checkboxes, which are not required but can be selected based on the form questionnaire.

  1. Required field - If this checkbox is selected, field users must collect information from the field.
  2. Add Image - If this checkbox is selected, Admin will be required to upload the picture while adding the widget to the form. Field users will see a label and an image for the relevant widget on the data collection interface.
  3. Display on map as Label - If this checkbox is selected, Admin, Analyst, and field users will be able to see the label on the map screen.
  4. Default to Last Collected - If this checkbox is selected, field users will not have to enter the information every time until they intend to modify it.
  5. Auto-Increment - This only applies to Number widgets that do not require incremental numbers to be entered. As data collection progresses, the application will keep increasing the numbers.
  6. Title Case - If this checkbox is selected, the first letter of each word will be in upper case, regardless of whether the data is collected in all uppercase or all lowercase.
  7. All Caps - If this checkbox is selected, user responses will be saved in all uppercase.

To create a field survey form, you can use any of the widgets listed below. You can also drag and drop widgets based on the form structure.

  1. Text - Enables the field user to enter text information such as Name and Address.
  2. Number - Enables the field user to enter details in Number format such as House number and PIN code.
  3. Date - Enables the field user to find and select the dates from the calendar.
  4. Single Choice - Enables the field user to select a single choice from a list of options.
  5. Multiple Choice - Enables the field user to select one or more choices from a list of options.
  6. Single Image - Enables the field user to upload a single image from the field, where they can capture photos with their smartphone camera. However, field users can also upload photographs from the gallery.
  7. Multiple Image - Enables the field user to upload multiple images by clicking photos using their smartphone camera. They can also upload photos from the gallery.
  8. Single Image Choice - Enables the field user to select a single image from the list of images.
  9. Multiple Image Choice - Enables the field user to select one or more images from the list of images.
  10. Signature - Enables the field user to upload the signatures of the respondent.
  11. Boolean Choice - Enables the field user to answer a question with 'Yes' or 'No'.
  12. QR or Barcode - Enables the field user to save the URL scanned from a QR code or a Barcode. This can be used in the area of interest, where the field user can scan the QR code that City Administration has installed. The QR code might contain citizen information in the URL format, which can be saved on the software against each entry of data collected.
  13. Audio - Enables the field user to record and upload the audio of the respondent.
  14. Video - Enables the field user to record and upload the video of the respondent.
  15. Document - Enables the field user to scan and upload the document of the respondent.
  16. Single Choice Auto Suggest - Enables the field user to automatically retrieve details associated to a given unique identifier. Admin can upload a CSV or Data URL and create widgets which resemble to CSV headers or columns. Created widgets must be mapped to the appropriate columns or headers. The 'Autofill' checkbox must be selected to fetch the form automatically.
  17. Calculate - Enables the field user to calculate the values entered in previously created number widgets using the operators (+, -, *, /,%). The Admin must create two number widgets to be mapped in Fields 1 and 2 of this widget.
  18. Time - Enables the field user to select the time when a clock appears on the screen.
  19. Rating - Enables the field user to enter the respondent's rating for the quality of service provided on the data collection interface using various methods including Star, Emoji, and Progress Bar.
  20. Mobile Number - Enables the field user to enter the mobile number where the ISD code is optional on the widget.
  21. Static Text - Enables the field user to store text as Static in the form and in data collection table such as City Name, Ward Name, Surveyor Name etc.
  22. Feature Detection - Enables the field user to find and locate various objects in the area of interest.

    Admin can use this AI-based widget to upload their own trained model or use a pre-uploaded model. Admin must specify the width and height of the image used to train the model after uploading their own trained model. An admin can choose objects such as a fire hydrant, automobile, motorcycle, bus, or traffic light when using a pre-uploaded model, and there are various options to select from the object picker which field user needs to identify from the field.

    Attention: It is recommended to upload the list of objects to detect in CSV or TXT format, together with the trained model in TFLite format.
